Parking Request
Use this hard copy form to notify Parking and Transportation Services (PTS) of a new regular employee that will not attend one of the weekly U-M New Employee Orientation sessions, temporary employee or other U-M appointments that are date specific (start and end date), such as Research Fellows. This information is required before the new employee can purchase a parking permit.
